This Consumer Health Data Privacy Policy explains how Seed Health, Inc. (collectively referred to herein as “Seed Health”, “we”, “our”, or “us”) collects, uses, shares, and protects “consumer health data” as defined by Nevada and Washington law, and the privacy rights and choices you have with respect to such data, in connection with our websites, https://www.seed.com/ and https://www.seedhealth.com/, our other affiliated websites that contain a link to this Consumer Health Data Privacy Policy (each, a “Site” and collectively the “Sites”), online services, products and services (collectively, our “Services”).
This Consumer Health Data Privacy Policy supplements our Privacy Policy and Survey Privacy Policy (distributed as required in connection with any surveys sent by us).
1. Consumer Health Data We Collect
“Consumer health data” is personal information that identifies your past, present, or future physical or mental health status. Seed Health collects consumer health data directly from you when you:
Contact Seed Health for support, questions, or feedback on one of our products, or for reporting an adverse reaction to a product; or
Request customized recommendations for our products, including by responding to our quizzes;
Participate in our surveys relating to your use of our products.
As described in our Consumer Health Data Privacy Policy, the information we collect depends on the context of your interactions with Seed Health. We may collect the following consumer health data:
Information about your health, such as medical conditions, symptoms, and menopausal status.
Information about your health goals, such as weight and fitness objectives and exercise habits.
Information about your history with dietary supplements and other medications, such as intake and preferences.
Inferences about the above categories of consumer health data.

We do not permit third parties to collect consumer health data over time and across different Internet websites or online services when you use our Services.
4. Your Rights
As a Nevada or Washington resident, you have the right to:
Access your consumer health data;
Delete your consumer health data; and
Withdraw your consent if we are processing your consumer health data with your consent.

Appeal: If your request to exercise a right is denied, you may appeal that decision by emailing us at privacy@seed.com.
5. Changes to This Consumer Health Data Privacy Policy
We reserve the right to modify this Consumer Health Data Privacy Policy at any time. If we make material changes to this Consumer Health Data Privacy Policy, we will notify you by updating the date of this Consumer Health Data Privacy Policy and posting it on our Services. We may also notify you in other ways from time to time about the collection, use, and disclosure of your consumer health data described in this Consumer Health Data Privacy Policy.
